LOG IN REQUIRED

This will allow you to track your service request status as well as re-open requests and use other services available through this system.

Instructions:

To Open A New Service Request:

1. Log In

2. You will be at the service request screen.

3. Complete and Save Request

To View the status of an open Request:

1. Log in.

2. On the menu at the top of the screen, click on Service Requests.

3. Click on My Open Requests.

4. Click on View to see the status of the request.

To re-open a Completed Request:

1. Log in

2. On the menu at the top of the screen, click on Service Requests.

3. Click on My Completed Requests

4. Click on View to see the request

5. Click on Re-Open to open the request

To fill out a New User MAC request form:

1. Log in

2. On the menu at the top of the screen, click on Service Requests.

3. Click on New User MAC request
